Abstract
Medical versus surgical treatment of the primary glaucomas has been a subject of continuing interest and some controversy.
Recent investigations support the clinical observation that long term miotic therapy, even with pilocarpine, may not be entirely innocuous, especially in patients with narrow angles.
Corticosteroids induce increases in intraocular pressure when applied topically to the eye. This pressure effect is in part related to the susceptibility of the eye being tested and is a genetic response.
The likelihood of a high corticosteroid response in the individual patient may be predicted from the presence of various conditions related to primary open angle glaucoma. They include a family history of glaucoma, diabetes mellitus, high myopia, cup/disc ratio larger than 0. 3 and intraocular pressure over 22 mmHg before corticosteroids.
Clinical evaluation of trabeculotomy was described.
Angiographic studies of glaucomatous eyes, especially optic disc changes were described. The plasma cortisol test was described.
The plasma cortisol test represents the first major difference found between homozygous high responders (gg) and patients with primary open angle glaucoma.
